Complaint Checklist

  • Identify the person or organization responsible for the alleged violation.
  • Write a dated timeline of what happened.
  • Preserve screenshots, emails, messages, notices and transaction records.
  • Keep copies of any privacy request or complaint you first sent to the organization.
  • Document the response, refusal or lack of response.
  • Identify the personal data involved.
  • Explain the harm, risk or privacy right affected.
  • Prepare witness affidavits if relevant.
  • Complete the NPC Complaints-Assisted Form or verified complaint.
  • Have the filing notarized where required.
  • Attach supporting documents in a clear, indexed set.
  • Retain a complete copy of everything submitted.
